ANNUAL CONFERENCE ART SHOW REGISTRATION FORM

All NAMI members are welcome to enter original artwork for the annual art show. Prizes will be awarded in the following categories. Please indicate the number of planned submissions in each category.

Please note the following conditions for the Art Show:
- The art show will be set up and available for viewing in the Art
Gallery on Friday and Saturday during conference hours. Art work must be removed from the Art Gallery on Saturday at 2:00 p.m.
- The artist is responsible for bringing and displaying art. Tables will
be provided. Any additional display items (easels, stands, etc.) may be requested but accommodations cannot be guaranteed.
- Winners of the art show will be announced during the Awards
Dinner on Friday evening
- All registrations must be received no later than Monday, October
23 to allow planning for show set-up.
- Conference staff and committee members are not responsible for
the safety and security of submitted art work.
- Maximum number of submissions per artist is five (5).

Your signature is required to certify your understanding of the
rules/expectations for the art show. Upon receipt by the state office
of your submission a form will be sent by mail for your signature.
