Please Note the following conditions regarding scholarships:
  • Scholarships cover conference registration expenses only. Meals are included in this fee. Hotel
    and travel expenses are the responsibility of the attendees.
  • All scholarship applicants are expected to seek assistance from other sources, including but
    not limited to affiliate funds, county funds, and personal resources.
  • Scholarship funds are limited; multiple applicants from the same family will be considered on a
    case by case basis.
  • Scholarship applicants must be members of NAMI Pennsylvania. You must submit affiliate
    information to verify membership.
  • Scholarship recipients must register for the conference using the Official Conference
    Registration form which will be circulated in September. A scholarship application IS NOT
    sufficient registration.
  • All scholarship applications must be received no later than September 15, 2006.
    Notification regarding your request will be made in writing no later than September 30,
    2006.
  • Limited number of scholarships are available for persons who are deaf or hard of hearing or
    family members.
  • Submission of this form affirms that I understand the expectations and conditions of this
    scholarship application.
