- All scholarship applicants are expected to seek assistance from other sources, including but
not limited to affiliate funds, county funds, and personal resources.
- Scholarship funds are limited; multiple applicants from the same family will be considered on a
case by case basis.
- Scholarship applicants must be members of NAMI Pennsylvania. You must submit affiliate
information to verify membership.
- All scholarship applications must be received no later than September 22, 2008.
Notification regarding your request will be made in writing no later than September 30,
2008.
- Limited number of scholarships are available for persons who are deaf or hard of hearing or
family members.
- Submission of this form affirms that I understand the expectations and conditions of this
scholarship application.
